Darker means more resistant.
- Tobramycin is the aminoglycoside of choice for Pseudomonas aeruginosa.
- In sepsis without culture data, use tobramycin, not gentamicin or amikacin.
Why: tobramycin is the most active aminoglycoside against P. aeruginosa and the only one with CLSI breakpoints for systemic infection (2023). Gentamicin has none; amikacin’s apply to urine only.
Susceptibility by organism
Gram-negatives, most active agents first
96Blue ring: preferred empiric regimen
SDD: covered only at the higher CLSI dose, so standard-dose coverage is lower than shown

Most of these agents are restricted formulary. Ceftazidime-avibactam, ceftolozane-tazobactam, meropenem-vaborbactam, cefiderocol, minocycline IV, tigecycline, eravacycline and omadacycline are released by pharmacy only with Infectious Diseases approval. Call ID when you send the culture, not after the result returns.

†Restricted formulary: ID approval needed.
‡FDA breakpoints (no CLSI breakpoints). Not for bacteremia.
*Amikacin for P. aeruginosa: urine isolates only.
Rintrinsically resistant; – not reported. CRE denominators differ by agent.
