UCLA Health Antibiogram 2026

2025 isolatesOriginal PDF ↓

Note: This antibiogram is based on RRMC and SMH data. For WV, it reflects outpatient isolates only.

  1. Find your organism, then the column for care location.
  2. The number is % susceptible — darker/redder means more resistant.
  3. Blue PREFERRED EMPIRIC REGIMEN = best first-line choice; red ID CONSULT = call ID first.
  4. SDD (susceptible-dose-dependent) means the isolate is only covered at the higher CLSI dose — not susceptible at standard dosing. SDD-flagged figures (cefepime, pip-tazo) include these isolates, so true coverage at standard doses is lower than shown.

Multidrug-resistant organisms

ID consult required

Most of what follows is restricted formulary. Ceftazidime-avibactam, ceftolozane-tazobactam, meropenem-vaborbactam, cefiderocol, minocycline IV, tigecycline, eravacycline and omadacycline are not released by pharmacy without Infectious Diseases approval. Place that call alongside the culture, not after the result returns.

Tables 6 & 7

It is recommended to start empiric fluconazole in clinically stable patients when BCID or culture results identify C. albicans, C. parapsilosis, or C. tropicalis.
